This office claimed to be a provider that if contracted with Anthem ******* Dental Insurance they would comply with allowed fees @ the discounted rates, with no copay to me. I have a flat yearly amount of $2,600, for in or out of network provider.s A pre-approval for the dental work was done on March **th, contracted with an unknown dentist I did not see, claiming he was in network. This office has dentists that (I suspect) are not employees but sub-contractors that come & go every few weeks. The first visit on Feb 12th with *************************, they honored the contract rate. I had an emergency visit on May 30th, a lady dentist not on any of the paperwork, was paid. Tooth #3 was on the preapproval of Mar **th, was meant to be repaired, a filling fell out leaving a hole, but had cracked with infection. They scheduled the following Monday June 3rd to extract ****** had them pull #**, that was on the Mar **th preapproval list. They billed $760.00, showing $368.00 from insurance & $392.00 remaining. I told them to bill ******* all of it. ******* paid them $618.00, that over paid them $250.00 they had already billed my private card.They started my work, they billed full amount, insurance pays allowed amount & then billed my private credit card for the difference, claiming the last 2 dentists were out of network? I should have no fee over what insurance allows.They billed my private card for $392.00 & was overpaid an extra $250.00 (above) from insurance. I can't get them to refund my credit card @ least the $250 (of the $392.00) & the remaining $142.00 (of the $392.00) should be billed to insurance from my balance of $1,696.00 left from the $2,600 I have for 2024, I haven't used.As I reviewed the ***************** on the 3 visits to this office, the dates were not correct & the fees were applied to different dentists out of network. They wont answer my calls & even **************** Dental cant get them to answer the phone.Can you help? *******
