I've been visiting Dr. ********* for several months. Typically, he sends appointment reminders a week in advance. I received a reminder on August 1, 2024, for my appointment on August 8, 2024. My goal for this appointment was to obtain a copy of the documents I signed at the outset and to continue my treatment.After texting him, I received a reply indicating he was at an emergency medical facility. While I understand that unforeseen events can occur, I believe a professional courtesy would have been to inform me of the cancellation ahead of time. When I previously needed to reschedule due to work, I was told to give at least two days' notice. Out of respect for his time, I left my meeting to attend my scheduled appointment.With the appointment not happening, I requested my documents via text, setting a deadline for a few days later. I was clear about my request and the timing, and his response seemed to convey annoyance. It's ironic, considering I was left waiting for an appointment I had driven an hour to attend. Subsequently, my agreement was terminated without my initiation. However, I'm still awaiting the documents that I initially requested. My texts appear to be blocked now, and my attempt to reach out via ******** received no reply despite being *******'s disappointing to have invested time and money into a service only to encounter such unwarranted behavior from the owner.I would also like to understand why I was charged a $75 account set-up fee when I have listened to his presentation and did not hear him mention a $75 set-up charge to two potential clients. This was not listed in his text of charges. I paid it, but I want to understand.Customer response

I sent in my screenshot information of his bookkeeping and a text message that went along with the first payment stating that I had made all the payments. At the end of the day. I need a copy of the documents that I signed with him thats what I want.Customer response

I have attached the form as requested. I just need a copy of the documents that I signed when I met with him for the first time. I think it was two documents. Thats what I requested and he has yet to give it to me and I have every right to those documents.Customer response
