      May 14, 2024 To Whom It May ************** be advised that the dental office of Dental Options billed for services that was not sent to my Florida Blue Dental Plan for processing or provided during my root canal and Crown procedure.On April 19, I went to the Office of Dental Options for a root canal and a crown. First they billed me $95 for the visit which should have been free according to contractual statement. Then they charged me another $813.00 after giving me a $100.00 birthday discount as a co-insurance for my root canal and crown which was covered by my insurance 100%. on May 7, your Florida Blue Dental agent ******** contacted Dental Options where they stated that they performed services not covered by my insurance including a Platinum Crown which I don't have, instead of a Ceramic Crown which was what they billed my insurance for.On May 10, 2024, I received another Itemized Bill from Dental Options for the full charges of the services they provided instead of the Allowed Amount between *************************************** when the Provider Dental Options had agreed not to bill me for the difference between the Provider's Charge and the Allowed Amount for the services they provided, and for other services that they did not provide to Florida Blue for processing and payment.On May 11,2024, I googled Dental Options on Yelp and saw that they had an average of less that 2.4 stars, and when I read the reviews, other people complained about being scammed by Dental Options be being overcharged for services the same way they did to **** am asking you to complete an investigation because I believe that I was intentionally overcharged in the amount of $ 908.00.I thank you in advance.******************************* ************ ID ********* Claim #***********

      ** There are additional services rendered in conjunction with the Root Canal that are not
      covered under your dental plan as it was noted on the treatment plan that was presented and
      approved by ******************** (see attached).
      April 19, 2024 D3911 Intraorifice barrier $180 (Non covered service under Insurance plan)
      April 19, 2024 D3999 Unspecified Endo-antimicrobial irrigation $ 320 (Non covered
      service under Insurance plan)

      ** There is an additional charge to upgrade the crown to Zirconia

      The total for non covered services was $992 (******************** paid  $813+$95).

      In conclusion, ******************** paid only for non-covered services at an agreed price as shown under columns PATIENT in the signed treatment plan and the rest of the services provided by our Dentists were paid by his Dental Insurance, as anticipated and shown under column PRIMARY in the treatment plan (attached).

      I signed a contract of treatment for invisalign by Dental Options in 2019. During covid, I had moved to ************ to the country with my family to try and avoid getting covid. During that time, Dental Options was mailing my retainers and I was doing my treatment that way. Once I came back to ***** in late *************************************************************************** with the wrong dental hygenist and that in turn made me miss my ortho appt. with the doctor. When I spoke with the Docter he was extremely rude and didn't care that the receptionist made a mistake and said that he would do my appointment but I had to wait an hour for him to come back from lunch. I had already waited an hour and a half in the reception area for the cleaning and then did the cleaning, so was there already for almost 3 hours. I had taken a half day at work to do the appointment. I was disapointed and left. They did not call to reschedule. Shortly after, I had to get an emergenct root canal at the same place and for months in and out getting treatment - the doctor assured me I would finish invisalign treatment once I got my new crown on. Now, they are refusing to finish my treatment even though I have a signed contract stating what is included in my treatment. They said that because I didn't call to reschedule that appointment that they messed up; "I abandonded the treatment" even though I've been at the office about 6 times in the past 6 months for the root canal surgeries and crown impressions and placement. I have been a patient there for 7 years and they have left the brackets on my teeth and told me that unless I pay for a whole new invisalign treatment; which I already paid in full, they will not take my brackets off or give me my retainers, even though in the original contract they were included. I have called over 10 times and tried to be patient, but they are unwilling to finish my treatment and made the insurance claim for the full amount.

      I spoke with them today and they finally agreed to help me with the retainers and to take my brackets off.
