      My son was admitted into the hosiptal on oct 3rd , i brought clothes on oct 4th , which was confirmed on camera , my son never received his clothes I called up there everyday from the 4th to his discharge date which was the 9th and they informed me they looked and cant find .. i spoke eith different nurse .. i also spoke with ***** located in unit 3 for minors !! I was referred upon discharge to get in contact with the risk management they handle this issue for reimbursement, i called ****** for several days never got call bk i then was referred to contact **** never got a call bk i then went up the ladder and got in contact with the *** that above both employees she informed me that **** would give me a call bk , he finally call bk stating was going look at cameras and look around couldnt find he ***** he can reimburse once i explain to him the items and prices which i can provide receipts he informed me that was too much he was going continue to look never heard back from him today i started calling around 9 am they kept transferring me to **** and ****** and the *** and i left vmails still havent got a call back . This is ridiculous i need to be reimbursed for my son clothes that the staff stole . Its going on a month and this issue still hasnt be resolved or i havent been spoke to about it

      05/15/2021 a nurse at Hartgrove administered me ******* (my roommate) 5mg of Ativan. My blood pressure dropped to low 80s, I felt lightheaded and off balance shortly after. I attempted to talk to the staff at the nurses station and was ignored by all parties. I asked what medication was given to me and if I could have names and documentation on the incident and was denied being told there would be none. Two staff members Zach and another tech spoke with me upon my issue but I do not know the other persons name. I called patients advocacy where I was sent to the voicemail of ****** and my call was never returned but she did come up to the unit on 05/17/2021 after two days of me calling getting her voicemail box. She too told me that it is not physical paperwork shes be able to give me to have for my own records. To this current day I still call and attempt to get a resolution but now risk management is telling me they have no recollection of my complaint. Which I made several while I was admitted. The document I attached below is from my insurance company who was billed every day expect the 05/15/2021 and 05/16/2021 which is the two days which I tried to speak with staff upon the situation.

      The facility is extremely dirty (GROSS); rooms have backed up showers with standing dirty water. My room had standing dirty water (old hair, grease, other particles) that passed my ankles. Was told by staff, "all the rooms have that problem, so there is no need to move you". Was told to take a shower anyway standing in that dirty, smelly old water. Day room was used for breakfast, lunch and dinner - had food all over the floor which was not swept for 3 days while I was there. The old food remained on the floor, we weren't allowed to wear shoes so if you weren't careful, you would step on old food and get on your socks which is tracked all through the facility. Unit was mixed gender and the men were allowed to make sexually vulgar remarks to women or just in general. The room monitors were young girls who often flirted with the young males. Some of the young men were allowed and encouraged by the young monitors to discuss their life of crime in detail. They often bullied the younger white males and made vulgar remarks to the younger white women, making one so uncomfortable, she refused to enter back into the day room unless it was time to eat. The scheduled "Groups" were NON-EXISTANT. My niece ordered items from ********** that sat downstairs from 1:30 pm until 8:40 pm (although I constantly BEGGED the staff to have someone get it several times). Several items were missing from bag (have receipt) - Missing items: Body Wash ($8.79); Lotion ($8.99); Vaseline ($6.49); 4 Word Search Books ($7.99 ea. - $31.96) - Total missing items: $56.23
