Around 9/18/23 a document from Global Life was dropped off at the Martin Luther King office for Marquerite LoPresti to complete The funeral office stated that the forms was ready to be picked up l get there only to be told by the doctor that they didn't have all pages and she would not be the one to complete form I provided the forms again to Maquerite to complete informing her per insurance she needs to complete the four page form cause she was his primary care provider (Kareem B*******) deceased When I asked when I could pick the forms up I was treated inhumane, causing more grief and anxiety, I was refused to speak with the manager and supervisor stating they were gone and would not be in office for two days This facility needs to be reported to Medicaid and Medicare and this doctor needs to be removed I just need the forms to be completed like it should be and treated like a human I am requesting your help in completion of my documents The deceased is Kareem B******* dob:******* Dod:******* Please help me I am a senior citizen. Due to my age and grieving I was taken advantage of by treating me inhumane
