I have been working with my nurse practitioner *************************** at Dr. ********************** Rosenman' office since February 8th, 2023, to receive an accommodation for my job. After finally obtaining the initial documentation from her, my employer now requires a time of duration for my accommodation, which my nurse practitioner failed to provide. This was requested several weeks ago, to no avail. My accommodation case manager faxed ******* again on March 28th, 2023, asking for a duration time for my accommodation. There has still been no response. In fact, because no one answered the phone as usual, I drove to said facility on April 5th, 2023, to request an update from ******* on the aforementioned documentation. Her receptionist, *********, stated that ******* was double booked that day and could not speak to me for even a minute, so I left a message asking ******* to call me with an update on the documentation. Two business days later, I still have not heard from her, nor has my employer received the needed documentation. This accommodation is crucial to my livelihood, and it is imperative that ******* expedite this process within her means. Thus far, she has been sorely lacking in the efficiency and responsiveness expected within standard business practices.Initial Complaint

I was notified over a month ago that I would be receiving a $120 refund from Dr ********************** **********************. The office manager ***** and an assistant named **** and a receptionist named ******* have all interacted with me regarding this issue. Every single week, I have checked up on the status, and still no refund, and no one knows the reasons for this delay, other than claiming that Dr ********************** is hardly ever in the office, although office workers verified that he has been there several times during the past month.
