      This has been a long battle and I have to warn others of my experience. Steer clear of Liberty Dental if you want peace of mind. I contracted Dr Sam and Dr George to do hybridge implants and from Day 1 there have been nothing but issues. The first red flag was them trying to bill my insurance for cosmetic dental services that I took a loan out for. Donna in the office Claims they were *** errors but not over 20 insurance claims are errors. Now my insurance is red flagged. Secondly the upper hybridge never fit properly and literally dangle on one side. I am unable to work due to the poor fitting. Dr George placed the initial implants and then left Liberty Dental. After I informed liberty dental of issues Dr George came back to liberty dental on my behalf and placed additional implants. My services have never been fully completed as the upper still is not fitting properly. NOW Dr Sam tries to pawn me off to a Periodontalist the consultation is a $122 mind you I've already spent over $40000 with liberty dental and my implants are not fitted properly. Dr Sam became very aggrumentative and demeaning after I explained I was not receiving the services I contracted. I visited Dr George at his new practice and he agreed that my treatment is not complete and agreed to reach out to Dr Sam to coordinate via text while I was in the office. Fast forward now in order for Dr Sam to further continue treatment he's making me sign a form basic***y releasing him of any issues being that he never properly fitted my upper high bridge implants. There were always issues. The form is also stating that the reason I refused to see the Periodontalist is because of costs that is not the case I'm refusing to see the Periodontalist is because I never signed up for these additional costs that were not discussed in my initial treatment with Dr Sam. My treatment plan consisted of Dr George placing the implants and Dr Sam placing the hybridge. This entire ordeal has caused me extreme anxiety and depression not to mention postporing future plans. My anxiety towards dentist has really increased. When you do not agree with doctor Sam he immediately becomes demeaning and unprofessional and it's very argumentative. All I want is the services and treatment that I paid for.

      The patient has signed a consent form with a recommendation of an upper over-denture by the Oral Surgeon. The patient was recommended an over-denture on her upper by the Oral Surgeon due to her jawbone condition. An upper over-denture will have a higher success rate than the upper Hybrid Bridge with her existing jawbone condition.
      We are doing a $60K treatment plan for $40K. ***** is reluctant to sign a new consent form that she needs to see a Periodontal Specialist for bone grafting. It is recommended the patient sees the Periodontist for bone grafting of her upper jaw to increase the success of her upper Hybrid Bridge. The first Hybrid Bridge failed because of her jawbone condition.
      The patient has refused to see the Periodontal Specialist, due to additional costs that she refuses to incur, as she does not believe she needs to see the specialist.
      We are not disagreeing the patient’s wants, we are willing to make another Hybrid Bridge with more implants, at no additional cost, as long as the patient is aware of the increased risks that this procedure may have in the future.
      We are simply asking the patient to sign the consent for additional implants as well as making a new upper Hybrid Bridge, with the patient aware of the guarded prognosis, life expectancy of another hybrid denture.
      Our goal has always been in the patient’s best interest and well-being. We cannot refuse the patients wants and desires as long as the patient is aware of the risks involved.
      ****** ***** is still under treatment and her plan has changed. When plans change, so do the terms and conditions. The new conditions necessitate new consents and that is the standard of care. *** she must do is sign the new consent form and she can continue her treatment.

      Better Business Bureau: I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.I would like a signed copy of my treatment PLAN that was agreed upon at my consultation..not a consent form that was altered. They only have a consent form where a wording "overdent" was not previously there and clearly looks edited. The oral surgeon Dr George whom left Liberty Dental confirmed that I have enough bone for the hybrid on 10/4 at his new practice. He confirmed my treatment is not complete. The treatment plan I agreed and signed to was $40,000 for upper and lower hybrid. Liberty Dental had me finance $40,000 now they are saying it was $60,000. The treatment plan signed with myself, ******* **** and Dr. Sam can't be produced. This signed agreement will clearly show what was initially agreed upon. Liberty Dental continues to try and bill my insurance for cosmetic surgery and all claims keep getting denied. Liberty Dental will not provide me a signed copy but want me to sign off on a Hold Harmless agreement in the middle of my treatment, this is unethical. This is all because Dr. Sam doesn't have anyone at his office to perform dental implants so my treatment plan changed. My upper hybrid has has issues because additional implants are needed per Dr. George. My visit on 6/3/23 confirms ongoing treatment with hybrid NOT overdenture.  Regards,  ***** *****

      Dear ****** There has never been any alteration in any form. You have to stop your false accusations if you wish to continue our care for you.The consent was hand written by Dr George and signed by you. It was scanned into your records by the office on that day.The bottom line is that we are agreeing to do the second upper Hybrid bridge as long as you sign the new consent that was adjusted and amended as per your request.As long as this consent is signed then we can keep your November 10th appointment at noon.Again, there has been no alteration or change made to any consent. Also, please feel free to send in any original treatment plans that you have.We await your signed consent and are anxiously wanting, like you, to give you what you want.Thank you

I would like a copy of the Initial treatment plan. The upper hybridge never was fitted properly due to not having enough implants to support it. Why am I being required to sign a new treatment plan if the original one was never fulfilled.
