My grandmother was admitted to Montifiore for PT/OT for her hip after being diagnosed with metastasized cancer. This was the absolute worse care ever! Along with being riddled with cancer she also suffers from dementia. When she arrived, she tested positive for covid so precautions were placed with the door closed 24/7x10days. There were multiple days where staff had not checked on her for more than 5 hours at time. They did not have the proper medications to administer. Not one time were vitals taken. There were ZERO fall precautions including no bed rails or bed alarm. She was left sitting in a chair for hours at time, with the door closed. She almost fell while aid was assisting her back to the bed. Aid could NOT manage on her own. They did NOT have it documented that she was a fall risk nor was she identified with an arm band or signage on the door. There was multiple times were the call button was pressed and it took staff hours to come and check or a family member had to physically go and get staff. One night she was not checked all night with the door closed; the aid told family member she "didn't know she was on her patient list to follow". They brought food but didn't have the strength to feed herself so she was only fed while family members were there. Supervisors made aware, all they offered was an "apology" and said they were "short staffed" which is NOT an excuse! 10/21/23 I found her with tachycardia with HR 157 and saturation at 87. The nurse did NOT even bring o2. Paged doc on call with no response for 2 hours. Sent to UH ER to be found with pneumonia and bladder infection. This was the WORSE care ever received and highly disappointed. My family would never recommend a place like this. If I did not have medical background, she probably would've died at Montifiore. Still have not been contacted by the doctor or Director of Nursing. Family is willing to get legal representation involved for NEGLIGENCE! Proper care could've prevented this outcome.
