Please keep all receipts for at least a year. Kleiman Evangelista Eye Center is ***** when it comes to business/billing keeping. ********** ******* in billing is very unprofessional and rude.*** ****, *** OH, *** ******* and ***** are wonderful people working for a TRASH company.Please see the attachment for bank transactions that I indeed pay for services rendered by Kleiman Evangelista Eye Center.I should not have to go through this process to prove I paid. Apparently there is a problem with your IT department and Billing system. I will be leaving a review on your website. You all are trying to cause a negative image on my credit and not that's not good business practice. Shame you and your company for claiming I was being deceitful when you can not go pass without being checked in and paying first. The power of SOICAL MEDIA will be my source of speaking out against Kleiman Evangelista Eye Center.**********, you very nasty with me on both phone calls. It is you duty to email clients not them emailing you. You were not concern nor apologetic in either conversation. Im making a complaint about the fact that I scheduled a simple eye exam. I asked specifically on the phone if they accepted our Aetna in network insurance for a simple eye exam. The girl said yes. I asked again to make sure. She says yes. I proceed to arrive today with them informing me that they dont take our insurance except under medical. I called my Aetna while the jerk of a girl was insisting that they only except medical and that I have not met my high deductible! The Aetna representative tried to inform them that we DO NOT have a deductible for a yearly eye exam!!! I am so mad at this unprofessional, joke of a place!!!!!Initial Complaint

I have not received the promised follow up to my concerns and complaints dating back to a visit at the Southlake office of the Kleiman Evangelista eye care practice in Southlake ** on December 18th, 2023. After insisting on knowing prices up front for fittings, I was given 3 different prices, including one for normal fitting, one for astigmatism and one for multifocal lenses. (There was no mention of mono focal correction). I accepted the terms because I knew I wasnt getting multifocal lenses, the most expensive fitting (a multifocal lens provides a gradient of vision correction in a single lens). Later, I was charged the highest fee for what the practice referred to as a mono focal lens fitting which, as I've already indicated, wasn't even mentioned by the employees when I asked for the prices up front. In addition, when I was given lenses to try on, I was surprised that I was offered one for near vision for my left eye and one for distance for the other. I asked if it would cost more to have one lens for near vision and another for distance and was told, no. I also was not told that having two different strength lenses would be considered a multifocal fitting. I left without paying (and without a prescription) because I was mislead, and because I never requested the two different corrections. Please let me know how I can go about rectifying this issue. I either need my contact lens prescription (for nearsightedness for both eyes at the astigmatism rate) or simply for this practice to no longer bill me so that I can get my prescription somewhere else. Also, please note, I was required to pay $35 at my original appointment, and I'm still not sure why, because the appointment was billed as a doctor's visit for my cataracts, which is completely covered by ********* My vision was checked during this visit, so I would think that that $35 should apply to my fee for the contact lens prescription.Initial Complaint

In 2013 my boyfriend (now ex-boyfriend) *********************** (name was ******************* at that time) went to Kleiman Evangelista to have our lasik procedures done. I opted for the 1 lifetime enhancement per eye "warranty" and paid the extra money for it. Now, 10 years later, I called them to see how I could go about using mine and was told that since the equipment they're using is different than 10 years ago the "warranty" wouldn't be honored and I'd have to pay for it. That's completely and totally absurd. I would like the money back that I paid for the lifetime enhancement option for both me and ******************* (I paid for both of our procedures) since it was never used and now won't be honored.Initial Complaint

I had lasik done on both eyes July 2012. I paid $3500 for the procedure and 1 lifetime enhancement per eye. I recently contacted them for an enhancement and I was told they changed management companies and no longer honor the their agreement. November 28, 2022 I need your assistance, I would like to file a complaint against, SAME DAY SURGICARE.Theyre billing me for $204.71 that I dont feel that I owe. They did not provide me with the best of care. Prior to surgery they pretended as though they were aiming to please, and they were going to provide this outstanding service.I dont feel that I received the wonderful lenses that they had claimed that I was going to receive. I feel that I received subpar treatment, because I couldnt afford to purchase their lenses that cost in the thousands of dollars. After surgery, I began to experience all sorts of issues, burning pain, debris in my eyes, swelling, vision issues, among other symptoms, and when I called their office for help, not even the doctors seemed to care. I was in pain, and I kept calling for an earlier appointment to see the doctor, they kept denying me an appointment. I called and asked if I could speak to the doctor, after calling for several days, they seemed annoyed with me, and finally I was able to speak to the female doctor that they had assigned to **** explained about the pain and issues that I was having, she did nothing, I was fearful of losing my eyesight. And she did not tell me to come in to be seen, she told me to wait for my follow up appointment which was over a month away. On my previous visit, prior to the phone call, I saw the surgeon that performed my surgery, kickin it with the nurses at the nurses station. I had asked the nurse that was walking me back to the patient room if the surgeon could please come talk to me. They did not allow me to speak to him, when I walked out of the patient room, he looked at me as I exited.Its been a year, and my eyes are still not right, I am considering contacting my insurances to see if they can investigate this matter, after what Ive been through, they should be ashamed to want any more money from me. This patient was provided safe and effective care during their stay with Same Day Surgicare. ************* benefits were filed appropriately and the patient was obligated to pay a co-insurance balance per her agreement with her insurance provider. An estimate was provided to her prior to her surgery day and her EOB (explanation of benefits) should have clearly stated that information for her to view post surgery. It seems most of her complaint is with her surgeon's office (Kleiman Evangelista) where her preop and post op care took place, as we do not provide that service here, only the surgical portion of her care.

August 24, 2022 My wife had a laser surgery in 2007 and received a written offer to have a yearly follow up with a cost $50 each time per life.Today, she was scheduled for this follow up and was told that the cost would be $180 and any previous contract would not be honored.This is breach in the contract.I need to mention that she has had this check up done several years at this cost.I need the company respect the contract.Initial Complaint

My eye doctor referred me to Kleiman when they were not able to resolve a chalazion/stye on my lower lid with 2 different antibiotics they prescribed. They told me that it would have to be cut/scraped out & Kleiman were "eyelid specialists". During my 1st visit 9/1, the doctor @ Kleiman examined my lid and prescribed me an antibiotic that my eye doctor had previously prescribed & told me to come back in 2 weeks. The 2nd appt 9/15; I was examined by a different doctor. He prescribed me a different antibiotic. I told him that my eye doctor had already tried the 2 antibiotics that my eye doctor had prescribed & they didn't work & I needed to have the chalazion surgically removed. He told me I needed to take it again & return in 2 weeks. On 9/28/21 they did the eyelid surgery & told me to come back 10/29/21. My eyelid was bruised & swollen from the surgery but the dr. told me that would subside. The bruising faded-but the swelling remained. (I have pictures of both) I cancelled my follow up appt on 10/28 because I was concerned about Kleiman's practices. I have a reimbursement acct for my out of pocket medical expenses & that was drained by Kleiman. I had to wait until 1/1/22 when it was re-funded & made an appt in 2/2022 with an eye specialist I had seen several years ago. He determined that while Kleiman "scraped out" the chalazion; they did not remove "the infected sacs" on either side of it & that was why it never healed. He prescribed a different antibiotic will have to perform surgery a second time. I have been using an electric heating mask to try and decrease the size of it so the surgery will be "less traumatic" to my eyelid. In March 2021, I received a "past due" **** from Kleiman for $75 which I found out was the amount left over from my last visit that my reimbursement account couldn't cover. This was the first time I heard from Kleiman since 9/28/21. I believe they should credit this amount because they did not cure the chalazion.Initial Complaint

I originally had Lasik completed by ******** Evangelista in 2017. One of the reasons I chose ******** was their LIFETIME enhancement guarantee which stated unlimited enhancements for the first 2 years were at no cost. Past the 2 years, enhancements were subject to a fee of $700. This enhancement guarantee was provided to me in writing by virtue of a signed contract between myself and KE Eye Centers. On 3/2019, I had an enhancement completed on my left eye and was not charged as per the contract. I have continued to use KE Eye centers for my annual vision review. Most recently, I called KE and requested to schedule a consultation for an enhancement due to a decline in the vision in my right eye. I came in for an appointment which was NOT a consultation as requested, but instead a regular vision appointment. During the appointment I reiterated my request for an enhancement. I was advised by the doctor that my right eye did not appear to qualify for a Lasik enhancement, but that I could potentially have PRK done instead. She stated she would email the surgeon. No one called me, so I followed up. Again I was told they would forward to the surgeon. Today, I spoke with ******* and office manager ***** who advised they would not honor my contract and if I wanted the enhancement I would need to pay $1500. I asked for the language in my contract that gave them grounds to not honor the terms and was advised the business was sold. I again asked for the language and was told the original doctor was not there. I again asked for the contract language and ***** confirmed what I already knew, there is no language in the contract that warrants the increased price. The business may have been sold, but they are still holding themselves out at KE Eye centers per their website and branding. KE needs to honor the terms of the contract signed between KE Eye Centers and myself. Side note, KE failed to disclose to its patients that the business was sold and doctor was no longer practicing.
