      On March 2, the above business determined my wife would need two infected teeth to be extracted. I provided the office staff with my primary medical(GEHA) and dental(MetLife) insurance information. After this was processed I was informed that payment due the day of service would be in the amount of $650.00 (which I paid) and it was estimated between my two insurances, the remaining cost of service would be covered. After insurance claims were processed, I received multiple bills from the business. The bill states I am responsible for $315.15 However, I told them that both GEHA and MetLife have notified me that none of the claims submitted reflected a portion of the treatment, which is coincidentally in the amount of $650.00. As was explained to me by the insurance companies, all facets of the treatment should be submitted for proper processing.I notified the business on 10/31 that GEHA informed me they had reached out to them on 3/15/2022 and requested they submit the claim to the correct point of contact for non-******** procedures, which in this case was **************************** services. The business claims they submitted via fax to UHC 3 times, however, the only contact info I know of is via a P.O. Box. I called GEHA on 12/19 and they confirmed the business still has not sent the claim to UHC for the $650.00 charges in question.The last bill I received now also includes an additional 1.5% finance charge which is unwarranted as the business has failed to file the claim correctly/completely. As a result I will not pay a balance(assuming there is still one) until after the insurance claims are filed correctly.I have 2 requests:1. The 1.5% finance charges should be removed 2. The business needs to file the $650 treatment portion for processing to UHC:EDI Payor ID #***** United ***************** Services ********************************************************** and to MetLife for secondary claim:MetLife Dental Claims ***********************************************
